The ED Care Coordinator is responsible for providing case management, care coordination, transitional care interventions and community resource referrals to Copley patients. This position will aim to develop a therapeutic relationship with patients, while assisting them through the process of better health by providing support, encouragement, and education. Work with patients and care team(s) to set goals to achieve desired clinical, financial, and resource outcomes. Must understand family systems, community resources, insurance, regulatory and legal system issues relevant to smooth transitions for patients and families.


  • Using Motivational Interviewing techniques, the ED Care Coordinator will consistently screen patients in the ED for needs related to social determinants and behavioral health, and provide any necessary resources and/or referrals. Share documentation of screening results and actions taken with other care coordinators and appropriate providers.
  • Provide coordinated care through communication and collaboration with internal and external care team members to provide continuity of care for patients entering and leaving the Emergency Department.
    • Forward Social Determinants of Health Screening to the Primary Care Provider
    • ED Care Coordinator will accept referrals from outside partner organizations to provide support to patients entering the ED to arrange for services upon discharge.
    • Participate in care teams to collaborate on supporting patients with frequent ED visits.
    • Attends, participates,contributes and facilitates in CHT meetings, LCMT meetings, and ACO care team conferences as needed or assigned.
    • Follows up on referrals to assess patient participation with referral agency.
  • ED Care Coordinator will make referrals to inpatient case managers for patients admitted .
  • ED Care Coordinator will communicate about patient needs, and actions taken so far to address the needs.
  • ED Care Coordinator will maintain status as a certified VT Health Connect Enrollment Assister and will enroll uninsured patients either while in the ED or prior to discharge.


Education Requirements:

  • Bachelor's Degree
  • MSW (Preferred)

Experience Requirements:

  • 3 years previous experience in a healthcare setting

License and Certification Requirements:

  • LICSW preferred
