Overview

Under the supervision of the Director, the Staff Pharmacist is responsible for assuring the drug therapy needs of the patient are met. There are five major areas of responsibility for the staff/clinical pharmacist. clinical pharmacy, drug distribution, teaching, administrative duties and conducting research. Compliance with all applicable State and Federal laws, regulations, and policies governing the provision of health care.

Responsibilities

  • Assists the nurse with routine updating of the patient’s medication administration record (MAR) as appropriate
  • Compounds medications for floor use and hospital Pharmaceutical Service Department use.
  • Develops and/or revises interdepartmental policies and procedures as requested by the Director of Pharmaceutical Services.
  • Dispenses and compounds or supervises the dispensing of medications and intravenous admixtures.
  • Inspects satellite pharmacy and/or medication storage areas on patient floors
  • Interprets and clarifies medication orders and applicable treatments directly from the Physician order sheet or order copy.
  • Is responsible for assuring that medication schedules are appropriate in relation to the patient’s ability to metabolize and eliminate specific drug compounds (i.e. amino glycoside kinetics, aminophylline kinetics, etc.).
  • Maintains patient records to monitor drug therapy for appropriateness and safety consistent with a high level of patient care.
  • Maintains pricing and location index of pharmacy drug formulary and monitors the preparation of accurate patient charges (i.e. billing)
  • Maintains records on controlled drugs.
  • Other duties as assigned in compliance with all departmental and hospital policies and procedures.
  • Participates in discharge rounds.
  • Participates in investigational drug and research studies.
  • Participates in the medication errors and adverse reaction reporting.
  • Participates in the selection of appropriate medication(s) for specific indications.
  • Prepares all IV admixtures and reconstitutes all lyophilized drug products when requested.
  • Provides discharge counseling to inpatients leaving the hospital with medications when requested.
  • Provides on-site drug information of all types to medical, nursing or other hospital professional staff.
  • Serves as an educator of hospital staff and students by preparation of clinical conference and/or “in-service”.
  • Supervises pharmacy technicians, pharmacy clerks and pharmacy interns and coordinates work flow

Qualifications

  • B.S. in Pharmacy degree or higher degree.
  • One year of experience in hospital pharmacy practice or have completed a hospital residency - desired. 
  • Registered pharmacist in the State of Vermont.
  • Familiar with unit dose, IV admixtures, computer skills, technical and clinical pharmacy practice. Must be self-directed and have excellent communication skills.
