Overview

Copley Hospital, located in Morrisville/Stowe VT,  is looking for a Cardiopulmonary Rehabilitation Clinician to join our team! 

 

As a member of the multidisciplinary team, the Cardiopulmonary Rehabilitation Clinician provides patient assessment, interventions, education, documentation, reporting and communications specific to Cardiac Rehabilitation, Pulmonary Rehabilitation, and Secondary Prevention Program needs. Performs other duties as assigned. Is compliant with all applicable State and Federal laws, regulations, and policies governing the provision of health care.

 

This is a Part-Time position (FT available).

Responsibilities

  • Provide group and individual patient and family education for any established CR and or PR education topics: -Exercise -Nutrition -Weight Management -BP Management -Lipid Management -Stress Management -Heart Function -Cardiac Symptoms -Risk Factors -Medications -Energy Conservation -Lung Function -Pulmonary Symptom Management -Supplemental Oxygen needs -Breathing Techniques -Infection prevention and Management -Other topics as required by specific patient assessment and needs.
  • Assess patient compliance with all medications based on patient reports, symptoms and clinical evidence: BP, HR, telemetry, lipids, BG, pain, Oxygen Saturation, weight, medications for co morbidities.
  • Assess need for and provide Tobacco cessation behavior interventions to promote tobacco cessation and long-term tobacco-free adherence
  • Determine appropriateness of using Heart watch with and teach patient the use of for the following purposes: safe progression from telemetry to self monitoring heart rate, teaching self monitoring, assessing patient’s accuracy in self monitoring pulse.
  • Participates in department and program staff meetings and quality improvement activities. Contributes to continually assessing and improving the quality of the programs. Promptly and appropriately responds to changes in procedures and policies. Proactively discusses procedural issues and opportunities for improvement with supervisor and other assigned individuals, in a professional and constructive manner.
  • Utilize the department computer system for telemetry monitoring and all documentation requirements, incorporating system upgrades. Participate in system troubleshooting and communications with technical support staff.
  • Follow department procedures and policies for patient registration and billing.
  • Complete a comprehensive patient Initial Assessment, and document in accordance with department policy. Perform and document these within the approved and allotted time frames.
  • Manage the exercise sessions safely while capturing all required documentation elements of the program. Monitor, direct, assess and progress the programs of 4-5 telemetry or non-telemetry patients in accordance with each individual Assessment and Plan, and based on patients' signs, symptoms and reports.
  • Develop patient individualized treatment Plan of Care in accordance with department policy in the time allotted.
  • Assess signs and symptoms of hyperglycemia and hypoglycemia and take appropriate actions.
  • Compile & submit Evaluation and Plan of Care to referring providers and Medical Director in accordance with CPR standards for assigned patients, in the allotted time frames.
  • Complete and submit monthly Progress Reports for assigned patients to referring providers and Medical Director, within appropriate time frames.
  • Compile and report patient blood lipid data in accordance with CPR standards.
  • Complete and submit Discharge Reports for assigned patients to referring providers and Medical Director, within appropriate time frames.
  • Communicate and interact with patients, families, colleagues, management & providers including Medical Director in a safe, professional, accurate, considerate, timely manner.
  • Employ Critical Thinking skills to achieve outcomes and solve problems.
  • Take personal responsibility for providing professional, organized & compliant care that meets patient needs.
  • Incorporate relevant safety and intervention skills in delivery of care.

Qualifications

Education Requirements

Physical Therapist: Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited school.

Exercise Physiologist: Bachelors' Degree in Exercise Science or Exercise Physiology with a focus on Cardiopulmonary Rehabilitation.

Registered Nurse or Respiratory Therapist: Associates Degree in Nursing or Respiratory Care from accredited schools.

 

Education Desired

Exercise Physiologist: Master's degree in Exercise Science.

All clinicians: Advanced training in Cardiopulmonary Rehabilitation preferred.

 

Experience Requirements

Three years experience providing patient care services in a healthcare environment in the fields of cardiac and or pulmonary rehabilitation, Acute care Physical Therapy, Respiratory Therapy or Cardiopulmonary Nursing.

 

Experience Desired

Experience performing patient assessments, interventions, education, documentation, reporting and communications specific to Cardiac Rehabilitation and or Pulmonary Rehabilitation. Use of EHR. Prior or current telemetry experience.

 

License and Certification Requirements

Physical Therapist, Registered Nurse and Respiratory Therapist must have current Vermont licensure without stipulations.

All clinicians: Current BLS & ACLS Certifications, or certifications acquired within 6 months of hire.

 

License and Certification Desired

Certification as a Clinical Exercise Specialist (ACSM).

Certification in Cardiopulmonary Rehabilitation from AACVPR.

 

Skill Requirements

In accordance with CMS regulations: Must be able to independently perform a comprehensive assessment of the cardiac and pulmonary rehabilitation patient. Must be able to supervise the telemetry monitored exercise of 4-5 individuals, while progressing the individual exercise programs, providing patient training in the use of all exercise equipment, assessing patient response to exercise, taking and interpreting all appropriate vital signs including but not limited to HR, BP, RPE, cardiac telemetry and pulse oximetry and documenting the exercise session. Interpreting 3 lead telemetry strips while patients are exercising in the program. Use of the computerized patient monitoring and documentation system. Cardiac and Pulmonary Rehabilitation education and communication skills with individuals and groups. Work and communicate collaboratively with all members of the multidisciplinary team, including the medical directors and referring providers, in accordance with AACVPR guidelines.

 

Skill Desired

Ability to establish and modify the exercise prescription based on stress test results, other diagnostic test results, medications, patient co morbidities and patient responses to exercise.
