Overview

Copley Hospital, located in Morrisville/Stowe VT, is looking for a Licensed Practical Nurse to join our team in the Multi-Specialty Clinic!

 

As a licensed member of the care team in the Multi-Specialty Clinic at Copley, the LPN will work in the General Surgery clinics to support the patients' plan of care. The Clinic LPN will provide general nursing oversight to patients during clinic visits, participate in in-clinic procedures, nurse triage, and referral and care coordination.

 

Requires LPN licensure in good standing.

 

This is a Part-Time position (FT available) for one shift on Monday, Tuesday, or Wednesday from 7:30am-4:00pm.

Compensation: Negotiable.

Responsibilities

  • Incorporates teaching in delivery of care and demonstrates competence in health promotion & disease prevention. Provides information to patient family appropriately, assesses for barriers to learning, uses colleagues as a learning resource
  • Coordinates care within the multi-disciplinary team, interacts in a professional manner, communicates directly, verbalizes scope of practice for self and others
  • Develops and maintains human care relationships & integrates the influence of the human experience of health, illness &/or healing. Adapts care for diverse populations, assists colleagues with care delivery, refers all concerns/issues to appropriate resource
  • Arranges physician referrals when appropriate, coordinating care with appropriate support agencies
  • Develops professional relationships with staff in order to enhance patient care and efficient clinical operations, acting as a role model for nursing support staff.   Abides by the policies and procedures of the practice, the Copley Health Systems Physician Practices and the Copley Health Systems Hospital
  • Is actively involved in creating educational materials and promoting educational programs appropriate for the patient population seen in the practice
  • Counsels and teaches patients on a variety of health care matters such as self-care appropriate to patient’s medical problem, preventive interventions, wellness programs, etc
  • Develops and maintains relationships with patients and adapts care for diverse populations
  • Supports and demonstrates organizational values, including compassion and respect for human dignity, commitment to professional Competence, commitment to a spirit of service, honesty, confidentiality, and good stewardship/careful administration
  • Complies with Copley and practice policies and procedures, and governmental regulations.

Qualifications

Education:  Required - LPN License

 

Experience: Desired - one year's practice as Licensed Practical Nurse. 

 

Licensure and certification:  LPN licensed in the State of Vermont.  BLS.
